Troubleshooting Guide for Comprehensive Assessments

Purpose

This guide is intended to assist the troubleshooting of Comprehensive Assessments that produce errors when finalizing. The steps below have historically been found to resolve most issues on their own, however in some cases a request will still need to be submitted to resolve the issue. These situations are outlined throughout the guide. Additionally, after each step is completed the comprehensive assessment should be saved to ensure that each potential issue is addressed when attempting to finalize again.

If a step produces an unexpected result (e.g. a saved field doesn't remain as set after saving), or you complete all the steps and are still unable to finalize, please reach out to the HHUNY Help Desk via a System Functionality Issue in Netsmart ticket and let us know what the issue is and/or what you have tried already.

Steps

  1. Check to ensure all required sections are completed:
    • You can check this by clicking the circled ! icon at the top right corner of the comprehensive assessment screen.
    • If you find that there are sections reported that need to be completed, complete all of the sections listed and save as draft. Then, back out of the comprehensive assessment and re-open it. The list produced by clicking the circled ! icon should be blank, but if there are any that still show up then please reach out to the HHUNY Help Desk for further assistance.
    • Once you do not see any missing required sections being reported, move onto step 2.
  2. Check to see that any new problems or providers are not already added to the chart, and add any that are missing from the chart manually:
    1. Look through each tab and review all entries listed as "New Provider" and "New Problem".
    2. For each instance of the above, click the dropdown to see if the provider is listed already.
      1. If they are, choose the existing entry.
      2. If they are not, add the problem or provider to the chart under the "Problems" or "Care Coordination" tabs respectively.
    3. Save as draft after each change. If you receive an error message when you save on any specific item, please reach out to the HHUNY Help Desk.
    4. Attempt to finalize the chart after all problems and providers have been selected to be one listed in the chart.
  3. If the comprehensive assessment does not finalize after completing step 2,  please reach out to the HHUNY Help Desk via a System Functionality Issue in Netsmart ticket and document the steps above that you have tried.
