Purpose of this Article
The purpose of this article is to allow a user to determine whether a note needs to be voided, or if an alternative resolution is recommended. Please note that the recommendations in this article are not absolute; if you wish to request a Note Void despite these recommendations, you may do so provided you understand that your chart may be reviewed in an audit.
Fields in Finalized Note that are Editable by a Care Manager Supervisor
Under typical circumstances, the following fields within a finalized note in Netsmart can be edited by a Care Manager Supervisor:
- Service Code (Billable vs Non-Billable)
- Type of Activity (Care Management, Community/Social, Health Promotion, Member Support, No Core Service Provided, Transition of Care)
- Contact Type
- Contact Status (Contact Achieved, Contact not achieved)
If the error made in the finalized note is in one of these fields, anyone with "Care Manager Supervisor" level access in Netsmart, can make the change and no void request would be necessary. However, if they receive an error while trying to make changes to these fields, please reach out to the HHUNY Help Desk for further guidance on how to rectify the error, as a note void may be necessary at that point.
Only an Error Resolution Contact Note is Needed
Under some circumstances, only an Error Resolution Contact Note detailing the error made in the initial note and the correct information will be needed. Below are a few examples in which the error made could be rectified by simply entering an Error Resolution Contact Note:
- Contact Date is incorrect AND the correct Contact Date is within the same month
- Note is a duplicate of another note
