Health Home Complaint Guidance

The use of this guidance is to maintain the health and welfare of all members during the provision of Health Home Care Management Services by HHUNY Network Care Management Agencies by identifying and investigating all complaints received from Health Home (HH) members and/or member designee and working to ensure appropriate actions are taken in response to the complaint and prevent reoccurrence of similar events.
